Instruction Sheet for the Firearms Applicant
FINGERPRINTING Applicants are required to call first for availability.
Fingerprinting is performed only on the following days when conditions permit and subject to availability.
Tuesdays from 10:00AM to 2:00PM. Please call (856) 914-3039 for availability.
Saturdays from 9:00AM to 11:00AM and 3:00PM to 5:00PM. Respond to Police Headquarters to check for availability.

Firearms Purchasing Identification Card:
A) Complete Form S.T.S.033. Application may be typed or printed
B) Answer all questions- must write (YES) or (NO) Do Not Circle.
C) A fee of $60.25 made payable by either Certified Check or Money Order to: New Jersey State Police- SBI (For State Processing)
D) A fee of $5.00 cash or check to: The Township of Moorestown

Consent for Mental Health Records Search:
A) The Firearms Applicant must complete Form SP-66. Failure to comply will result
in the dismissal of the applicant.
B) Applicant completes Part One Only.
C) Signature and date must be witnessed by a Moorestown Police Officer upon submittal.
D) Applicant shall be aware of disclosure in N.J.S.A. 30:4-24.3
Provides that all records of any individual's commitment to a non-correctional institution for mental health reasons shall be confidential and shall not be disclosed except in limited circumstances or with the consent of the individual. (This consent MUST be completed by the firearm applicant. Failure to consent requires denial or disapproval of the application). |

Application to Purchase Handgun:
A) Complete Form S.T.S. 033. Application to Purchase Handgun.Indicate amount of permits being applied for.One permit per handgun purchase.
B) Answer all questions must write (YES) or (NO) DO NOT CIRCLE
C) A fee of $2.00 cash or check to: The Township of Moorestown.
D) Complete Consent for Mental Health Records Search. (Applicant completes Part One only).
E) Upon approval, Permit is valid for 90 days. The permit may be renewed for an additional 90 days prior to the expiration within the first 90 days. Permit becomes voided after 180 days.

Application for Duplicate or Change to Firearms Purchaser Identification Card:
A) No fee for lost, stolen, mutilated cards or change of name/address and/or sex on Form S.T.S. 033. (Change of Name or Address Requires Old Card to be Surrendered).
B) Consent for Mental Health Records Check must be completed. (Applicant completes Part One only).
C) Applicant shall complete Form SBI 212A Request for Criminal History Record Information for a Non-Criminal Justice Purpose.
- A Fee of $18.00 is required and made payable to: New Jersey State Police SBI Payment is made
- with a Certified Check or Money Order. This is needed only if you are from another town.
D) Questions pertaining to special situations contact: Service Division, Monday - Friday from 8:30AM to 4:30PM at (856) 914-3036 or (856) 914-3035.

Permit to Carry Handgun:
A) The Applicant Must be at least twenty-one years of age.
B)The Applicant shall complete Application SP-642 Permit to Carry Handgun . This form shall be completed in its entirety and in triplicate (Three Original Applications)
C)The Applicant must submit four photographs (1 ½ X 1 ½ Square)
D)The Applicant shall complete Part One of the Consent for Mental Health Records Search- Form SP-66. This form may only be signed and dated in the presence of the Police Dispatcher or Desk Officer.
E)The Applicant must be fingerprinted by this Department on BOTH State Fingerprint Cards Form SBI-19 and a Federal Applicant card Form FD-258. A fee of $60.25 made payable by either Certified Check or Money Order to : New Jersey State Police SBI
F)A fee of $20.00 made payable by either Certified Check or Money Order to: The State of New Jersey
G) The Applicant must submit a Notarized Certification (Letter) of need, specific in content, under oath which:
- In the case of a private citizen shall specify in detail the urgent necessity for self-protection,
- as evidenced by specific threats or previous attacks, which demonstrates a special danger
- to the Applicant's life that cannot be avoided by means other than by issuance of a permit
- to carry a handgun. Where possible the Applicant shall corroborate the existence of any
- specific threats or previous attacks by reference to reports of such incidents to the
- appropriate Law Enforcement Agencies.
H) The Applicant shall list a complete description of each handgun he/she intends to carry.
I) There are additional requirements that must be completed. Please contact Service Division during normal business hours.

New Jersey Voluntary Registration of Firearms:
A) No Fee. Forms are available at the Police Desk Form SP-650.
B) Answer all questions (Type or Print)
Proof of Identification Needed New Jersey Drivers License or Passport
Background Investigation Be advised, The Moorestown Police Department will conduct a background investigation on all Applicants.
